Assistance for Members
Members of Group Plans: If you have not received benefits information or member ID cards, you can get assistance with your needs by calling our Member Services Center toll free at 888-855-5088.
Members with Individual Policies: If you have not received your welcome letter or member ID card, please talk to your insurance agent or call us toll free at 888-697-0683.
HMO Members: If you have not received your welcome letter, primary care physician selection information or member ID card, please call our HMO Member Services Line toll free at 877-299-2377.

If you have not received your member ID card and need prescriptions co-pay information, or have any other pharmacy questions, contact Prime Therapeutics at 888-275-3235.
