Blue Health Assessment (BHA) is a quick and easy online health evaluation program available to Service Benefit Plan members 18 years old and older.

BHA helps members identify lifestyle risks and take steps to improve their health. After you finish a 15-20 minute guided assessment, your answers are evaluated to develop a unique action plan just for you. You can save and print your individualized action plan, and share it with family members and health care providers.

There are 12 online coaching sessions to help you reach your goals:

Weight Management	Physical Activity
Smoking Cessation	Chronic Condition Management
Stress Management	Pain Management
Nutrition	Diabetes Management
Depression	Insomnia Prevention
Back Care	Binge Eating

In 2011, you have another good reason to complete the BHA. If you have Standard Option, we will credit $50 to your calendar year deductible for each person who completes a BHA, up to $100 when two adults complete the BHA under a family enrollment. The $50 must be earned and credited before you meet your 2011 calendar year deductible. If you have Basic Option, you will get a $35 health debit card to use for qualified medical expenses when you complete your BHA. Family contracts are eligible for two $35 cards when two adults complete the BHA.
